P54 RIGHT SINGLE CORONARY ARTERY AS AN INCIDENTAL FINDING IN ACUTE PERICARDIAL DISEASE

Abstract An 44–year–old woman was referred to our hospital for chest pain. She had no known cardiovascular risk factors but a history of previous acute pericarditis following pregnancy. At admission, she still pain improved by leaning forward. Cardiovascular examination and her electrocardiogram showed nothing pathological. On laboratory tests, there were mild elevation troponin T normal C–reactive protein. A transthoracic echocardiogram revealed only minimal pericardial effusion close the right heart chambers. X–ray generally normal. admitted coronary care unit with provisional diagnosis myopericarditis. Three days later, due persistence symptoms despite appropriate pharmacological therapy, it decided perform angiography. During procedure, selective cannulation left artery not possible, non–selective injection disclosed arising from sinus. sinus, single ostium visualized. The (RCA) great vessel, stenosis. anterior descending (LAD) circumflex (LCx) perfused RCA collateral vessels, both without lesions. Coronary computed tomography angiography confirmed its in sinus Valsalva agenesis main (LCA). physical stress echo ruled out inducible myocardial ischemia. anomalies are rare congenital disorders that usually diagnosed incidentally. Single (SCA) has been since 1903. It is an anomalous condition which SCA originates aortic orifice, either or ostium, gives rise entire circulation. Isolated very condition, occurring 0.024–0.098% general population. Right absence LCA least common type SCA, extremely reported incidence 0.0008%. association atherosclerotic disease, fistulas, mitral valve prolapse, bicuspid valves, tetralogy Fallot, transposition arteries, hypertrophic cardiomyopathy Takotsubo syndrome. We report this case because concomitant diseases never previously reported.
